Savo + Osi >>> Osi In 1L stage IV, EGFRmut: Better PFS and remarkable 44.5 OS for a poor prognosis population, with the cost of more MET-related side effects. High attrition to find MET-IHC3+. Wondering how this data compares to this subset of patients in the MARIPOSA trial. #ESMO26 #LCSM

ESMO 2026 | A cautionary tale for immunotherapy de-escalation! MOIO Trial (LBA10) – Phase III non-inferiority trial testing 3-monthly immunotherapy vs standard dosing in responding patients with advanced/metastatic cancer. 📊 Key results (N=308): • Median PFS: 21.9 vs 25 months (reduced vs standard; HR 1.35) • Risk of death: HR 2.0 (95% CI 1.1–3.9; p=0.0143) ⚠️ • PFS HR in urothelial cancer: 2.9 • Grade 3–4 immune-related AEs: 9.5% vs 10.5% ❌ Non-inferiority NOT demonstrated. ❌ No meaningful safety advantage. ⚠️ Signal of inferior survival with reduced-frequency IO. 💡 Take-home: Less frequent immunotherapy is not necessarily equally effective! Important: These are interim results with relatively small numbers and heterogeneous tumour types. #ESMO26 #MOIO #Immunotherapy #Oncology @OncoAlert @myESMO

Pembro monotherapy still has a role in selected pts (borderline ECOG PS or where a pt does not want to do chemo) but the control arm did not underperform. The final median PFS for Pembro mono in KN024 was only 3 weeks less than the control arm performance in PERSEE was it not?

Brilliant discussion from Dr. @BenjaminBesseMD at #ESMO26 on the Krascendo 1 study of divarasib vs sotorasib/adagrasib. Rightly points out lack of crossover in a study with a PFS endpoint as unjust. Excellent points all around. #ESMOAmbassadors

Congrats to Prof. @ToffartC on his very clear presentation of the results from Phase 2 part of the IFCT-2103 DIAL trial (de-escalation of pembro at 6 months). There was no detrimental effect (OS) at 18 and 24 months (93 first randomized pts).. #ESMO26

‼️ In first line treatment of advanced PD-L1 ≥50% NSCLC, PERSEE demonstrates a significant improvement of PFS with pembro-CT compared to pembro monotherapy regardless of PDL1 level of (over)expression. Practise changing trial. @myESMO @OncoAlert #ESMO26 👏👏👏 @decroisette

🫁 PD-L1 ≥50% has long supported pembrolizumab monotherapy — but PERSEE asks whether some of these patients should receive more upfront. @OncoAlert At #ESMO26, phase III PERSEE compared pembrolizumab + platinum-based chemotherapy versus pembrolizumab alone in first-line advanced #NSCLC with PD-L1 ≥50%. 📊 n=321 ➡️ Median PFS: 16.1 vs 6.9 months HR 0.56 (0.43–0.74), p<0.001 ➡️ Median OS: 50.2 vs 29.9 months HR 0.77 (0.57–1.05) ➡️ ORR: 77.5% vs 50.3% But the trade-off is substantial: ⚠️ Grade ≥3 TRAEs: 53.8% vs 19.3% 🎯 Why this matters: PD-L1 ≥50% may not be enough, by itself, to decide between pembrolizumab alone and chemo-immunotherapy. For me, the real clinical question is now: Which PD-L1–high patients need the deeper and faster disease control of chemotherapy upfront — and which can safely avoid the added toxicity? That is where tumor burden, symptoms, pace of disease, comorbidities and better biomarkers will matter most. Excellent work from @decroisette @BenjaminBesseMD @LGreillier @C_Ricordel @FGuisier and the PERSEE investigators. #ESMO26 | PERSEE – Phase III In advanced NSCLC with PD-L1 ≥50%, adding chemotherapy to pembrolizumab improved median PFS from 6.9 to 16.1 months (HR 0.56). The numerical OS difference was notable (50.2 vs 29.9 months; HR 0.77), but not statistically significant. However, we should remember that OS was not the primary endpoint! Grade ≥3 toxicity: 53.8% vs 19.3%. I think adding chemotherapy provides benefit, but at the cost of increased toxicity. We need to better identify the patients for whom this toxicity is worth it.
