📊 Prostate cancer treatment is shifting toward risk-adapted care.
From 2010–2023:
🔹 Low risk: radical prostatectomy ↓ ~58%→11%, while expectant management ↑ ~20%→78%.
🔹 Favorable intermediate risk: surgery ↓ as surveillance/expectant management ↑.
🔹 Unfavorable intermediate & high risk: definitive treatment remains key, with radiotherapy playing an increasingly important role.
The takeaway: radical prostatectomy isn’t disappearing—we’re getting better at selecting who truly needs treatment and avoiding overtreatment when it offers little benefit. @JAMA_current

🚨 PRIMARY2: PSMA-PET moves into the prostate cancer diagnostic pathway
📰Just out in @TheLancetOncol
🧬 660 biopsy-naïve pts w/ suspected significant PCa + PI-RADS 3 MRI or PI-RADS 2 MRI w/ high-risk clinical features
⚖️Randomized to [⁶⁸Ga]Ga-PSMA-11 PET-CT vs systematic transperineal biopsy.
🧭 PRIMARY score 1–2 → surveillance. PRIMARY score 3–5 → PSMA-targeted biopsy.
🎯 PSMA-PET triage was non-inferior for clinically significant PCa detection: 12% vs 16%
✂️ Nearly half avoided biopsy: 49% (163/331).
📉 Clinically insignificant cancer detection dropped substantially: 14% vs 32%.
⚖️ Big implication: fewer biopsies + less overdiagnosis, without losing significant cancers.
💡Caveat: follow-up ongoing; health economics + validation w/ other PSMA tracers needed

|GnRH antagonists give faster LUTS relief in prostate cancer
#PROSCA25 #GlobalForum25
Urinary symptoms matter in men starting RT plus ADT. Across multiple trials (Anderson, Mason, Axcrona), one signal is crystal clear:
🟦 Degarelix and other antagonists drop IPSS faster than LHRH agonists + bicalutamide
🟦 Benefit is strongest in men with high baseline IPSS
🟦 Improvement appears independent of prostate shrinkage
🟦 Likely linked to direct action on BPH cells and the bladder
🟦 Helpful during RT when LUTS often flare
Clinical takeaway:
When a patient needs quick urinary relief, an antagonist can give a noticeably faster improvement than agonist based ADT.

New State-of-the-Art Review: “The Role of Liquid Biopsy in the Management of Patients with Genitourinary Malignancies” - just published in the November issue of European Urology @EUplatinum by Dr. Benjamin Maughan and colleagues. It’s a great honor to be part of this amazing team, led by Dr.Shilpa Gupta.
Liquid biopsies offer many promising areas of potential clinical utility. We believe there is now sufficient validation to justify their use in patients with genitourinary cancers. We recommend somatic tumor testing for all patients with metastatic genitourinary malignancies, given both disease-specific and disease-agnostic indications. Such testing may also provide important prognostic insights that can guide patient counseling, though certain applications still require validation in prospective, biomarker-driven clinical trials.
We strongly encourage the extensive use of liquid biopsy testing when indicated, aiming to improve outcomes and support further research.
The clinical utility of liquid biopsies is expected to expand further as additional indications are evaluated and validated.
Future applications may include patient selection for adjuvant therapy in renal cell carcinoma and/or urothelial carcinoma, screening of high-risk populations, and new predictive assays for systemic therapy. Each new application will need to be validated in appropriately designed clinical studies as part of the ongoing effort to advance personalized medicine and improve patient outcomes.

🧬 Urine tumor DNA to stratify recurrence risk after atezolizumab in BCG-unresponsive NMIBC
In a secondary analysis of the SWOG S1605 trial, urine tumor DNA (utDNA) profiling with the UroAmp assay was used to stratify the risk of response to treatment with atezolizumab in patients with BCG-unresponsive high-risk non–muscle-invasive bladder cancer.
🔹 In patients with high-risk NMIBC that recurred after prior BCG treatment, UroAmp can identify which patients are at a high risk of cancer recurrence before and during the treatment with atezolizumab.
🔹 Patients with positive ut-DNA were less likely to respond to treatment and had a higher chance of cancer recurrence.
🔹 UroAmp supports therapy personalization through patient-specific molecular data
These findings highlight utDNA as a promising biomarker for treatment monitoring and risk stratification, potentially helping to guide therapeutic strategies in BCG-unresponsive NMIBC.
