For the cheap seats: an NHS that is free at the point of care is NOT the same as free universal healthcare You can keep the NHS free and still privatise the U.K. healthcare service

Can we please stop saying the NHS is in crisis? It was in crisis 10 years ago. Now it's broken.

Corporate penetration of the NHS’: Revolving door raises concerns over ADHD review. Instead of expanding NHS capacity, govts hand contacts to private providers making vast profits. The same providers meet ministers to influence terms of reviews. observer.co.uk/news/national…

Drug shortages, no screening re bone density at menopause or minimally a review of family history/hormone cancer history, no consideration of risk factors & mitigation. Broken promises on fracture services + failing health system.
Surgeon: "There has been a large national focus on delays for elective surgical care such as hip and knee replacements. However, delays for emergency surgery also result in significant and avoidable patient harm." The number of hip fractures is set to double in the next 10 years

And politicians who completely ignored an ageing demographic for decades. Whack the state pension age as high as you like, no accessible healthcare or (god forbid) GP lead primary care 🤷‍♀️

When they try to reschedule that appointment you have been waiting for six months for another six months your best bet is go in and talk to the admin team directly and very politely. Most people will just accept this and moan. #NHS #Health theguardian.com/society/2026…

The worst part about the Switch trial proving what we knew to be true about payload resistance is that ADCs are basically all we have as far as control of brain mets and leptomeningeal disease. For our CNS disease patients chemo just doesnt cut it. #ESMO26 #Breastcancer #ADC

🙏🏻 to drugs like Herceptin & Perjeta, fewer people w/ early stage HER2+ breast cancer even develop metastasis. Now w/ Kadcyla, Tukysa, & Enhertu more people w/ metastatic HER2+ disease are living longer. Enhertu can also be used in HER2-low but we need more research & options

With government attempts to cut our pay yet again in real terms (continuing decades of pay erosion), and no meaningful negotiation on our now 23 year old contract, this should surprise absolutely nobody. 🦀

Day 26 - Inflammatory Breast Cancer Inflammatory Breast Cancer (IBC) is a rare, aggressive breast cancer that often causes redness, swelling, and skin changes instead of a lump. More research is urgently needed to improve outcomes and save lives. Don’t ignore stage IV. #METober

Never say I didn't foresee it. So..here we go again. Open letter to @RCPhysicians @Mumtaz_Patel_1 @OGMustafa @BDChadwick If you are a Member or a Fellow- join in and sign. RT/Cascade/Sign as you can or wish to. The work to move the RCP out from the shadows into the light...continues. And no- I have zero need to anonymise my name- compared to the contributors of this daft protocol for PAs -post @lengreview Via @TheDA_UK @mmamas1973 @djnicholl @doctor_oxford @RobLaurensonD4P @DrSteveTaylor @dr_shibley dauk.org/rcp-letter/
Prediction time- and a bookmark for those who fancy it. There will be attempts to fudge the PA protocols and dilute by the pro “PA are just differently trained doctors” lobby. Absolute lock on that Leng will be under heavy pressure to be part of that Organisations such as Royal Colleges will try to manage it internally and “find a middle ground” (Colleges will mostly do as asked- unless the uproar is too much to handle) They will eventually come in the public domain There will be uproar and much debate as Fellows and Members see them. The NHS Long Term Workforce Plan will try- again - to shoehorn PAs in. The basic strategy? “Let’s find a middle ground”- based on a belief that people are tired of this and will accept. And a hope that enough has been given to the BMA - pay structures, IMG sacrifice etc for them to sign up As ever? The key sits with the supervisors. Will YOU as a Doctor- agree to taking lesser trained people on and put your liability on the line. And will you have the time once METR lands. Policy is one thing - involves much of politics Implementation? Needs wide support. Ask those who tried to force the whole PA project through first time around. @lengreview @RCPhysicians @rcgp @gmcuk @TheBMA @AoMRC @AoMRCTrainees @rcpsych @JanetEastham

COBR
Exc: Ministers have been secretly warned that some English reservoirs are at risk of running dry, hitting water supplies to homes in the run-up to Christmas. The government’s emergency committee, known as Cobra, is meeting up to twice a week to discuss the situation in the southwest of England and Sussex. A secret “reasonable worst-case scenario” was drawn up, anticipating that several reservoirs’ water stocks could fall to 0 per cent — dubbed “day zero” — within weeks. Officials warned that without action, supplies to some homes could be affected, along with disruption to the construction of a nuclear power station and operations at a prison. Steps have been taken by ministers and water companies to try to mitigate the risks, which insiders stressed were already starting to push back the “day zero” dates. Experts on the government’s Scientific Advisory Group for Emergencies (Sage) have also been brought in to help review water companies’ modelling for protecting stocks for the weeks ahead. Heatwaves over the summer left half of England in drought. Sussex and the southwest were the most severely affected. Wimbleball reservoir, which supplies more than 400,000 people in Somerset, has 13 per cent of its capacity, while Ardingly, which serves 280,000 customers across West Sussex, is at 17 per cent, figures from yesterday show. Reasonable worst-case scenario plans were drawn up by the government, setting out a “day zero” envisaging Wimbleball and Ardingly reservoirs’ stocks being completely depleted by December. Subsequently, the government privately said it was told by South West Water that it did not anticipate Wimbleball running dry due to measures it had put in place. South East Water is also taking action that has shifted Whitehall “day zero” forecasts at Ardingly into next year. Even if the reservoirs did run completely dry, water companies would probably be able to transfer supplies from elsewhere. However, significant concerns remain in government about water disruption to properties supplied by Devon’s Venford reservoir in early November. “Day zero” risk monitoring has also started for Cornwall’s Stithians and Drift reservoirs. Water companies were asked to model the impact of water-saving measures, which have been scrutinised by members of Sage. Ministers were told that shortages could affect daily operations at HMP Exeter and the construction of Hinkley Point C, a nuclear power station being built in Somerset. Normal levels of rainfall over autumn and winter will not be enough to counteract the effects of the drought, which was extended to cover Yorkshire this month. Officials have calculated there will need to be 120 per cent of long-term average rainfall over the next six months for a nationwide recovery.

The #NHS also called the “jewel in the crown” Now- Built on a model of “don’t come to see us” And if you do? “Please leave quickly” Whether it be primary care or specialist care It’s called demand management OR not enough resources to deal with the need of the population.

According to @JatinderHayre_ #OurNHS is being 'macdonaldised'. They think that if we appear to have been treated quickly we'll believe we're being cared for, despite any subsequent harm which may have been missed.

Quality of life 👇🏻👇🏻
Overall survival matters, but it’s not the ONLY measure of meaningful treatment benefit! For people living with #MBC, delaying progression may mean more time feeling well, maintaining independence & living life. Keeping cancer under control matters, even when it doesn’t mean living longer! #bcsm
