Grand Rounds tilted “New advances in immune-mediated podocytopathies: Is anti-nephrin the new anti-PLA2R?”
👩🔬 Key Conclusions
•🧪 Anti-nephrin antibodies confirmed as correlating with disease activity and likely involved in GCW alterations
•🧭 First biomarker available for diagnosis/monitoring in children + adults; may help prevent post-transplant relapses
•🧫 ELISA assays for clinical use coming soon (Euroimmun, bridge ELISA for research)
•🧩 New concept: Slit diaphragm antigens (nephrin, podocin, NEPH1, NEPH3) drive autoimmune podocytopathies
❓ Key Open Questions
•🔮 Can anti-nephrin antibodies predict steroid dependence/resistance?
•📈 Is there correlation between antibody levels and outcomes?
•🔁 Can they predict relapse?
•🧬 What are the antigenic targets in anti-nephrin negative patients (esp. primary FSGS)?
•⚠️ Are all anti-nephrin antibodies pathogenic?
•🧠 Other permeability factors yet to be found?
•⏱ Why is remission so fast after steroids?
•🧾 What is the mechanism of immunization?
•🕰 Perspective: “18 years after anti-PLA2R discovery, we still do not know everything!”

Membranous nephropathy has undergone a paradigm shift due to the discovery of unique MN antigens.
MN has gone from idiopathic to ➡️ primary vs. secondary ➡️ where an antigen can be detected in ~80% of MN.
This is a comprehensive review of each antigen.

Important new arguments for the discussion, which B cell-targeting therapy to involve in the treatment of autoimmune diseases. May be even more important in alloimmunity? ard.eular.org/article/S0003-…
Not all drug related TMA’s are made equal. But for a subset (I.e. VEGFi, Gemcitabine), terminal complement blockade allows continuation of the offending drug, with important implications for progression free survival in the long term.
Complement Inhibition Enables Anti-VEGF Receptor Therapy in Renal-Limited TMA. More options to keep patients on life-saving therapy.
