Hypervirulent K. pneumoniae (hvKp) represents a distinct pathotype historically associated with a hypermucoviscous phenotype, commonly detected by the positive string test. Importantly, this test is not a reliable marker of hvKp, as it is neither sensitive nor specific, leading to misclassification when used as the sole defining criterion.

“What’s in a Name? The Rise of Klebsiella pneumoniae: From Friedländer to Klebs”
This is my first publication as first author, and having this journey with my mentor, co-author, and friend, Dr. Spellberg, makes this milestone even more special.
K. pneumoniae is an organism I deal with almost every day—often as a notorious MDR pathogen. Writing this article gave me the opportunity to step back from the daily clinical challenge and explore the fascinating story behind its name.
From Friedländer → Klebs → K.pneumoniae.

2年ごとの便潜血検査で、50〜69歳の大腸がん死亡が50.1％減った。
スペイン約220万人を21年追った研究。便潜血を受けることで、大腸がん死亡が10万人あたり39.7人→19.8人まで減らすことができた。
便潜血は2日分のうち1日分だけ陽性でも必ず精密検査（大腸カメラ）。
・痔だったかもしれない、月経だったかもしれない
・もう一回検査して陰性なら大丈夫
・来年陽性だった受けよう
は全部よくあるダメな例。
大腸がんの家族歴がある、肉が多い食生活など、もともとリスクが高い人はなおさら。大腸がんは日本の罹患率NO.1。早期なら予後は良好だけど、進行してしまったら非常に厳しい。便潜血は超大切な検査だけど結果をスルーしている人がとても多い。1回でも陽性なら必ず大腸カメラを受けて欲しいなといつも思います。

高用量インフルエンザ HA ワクチン「エフルエルダ®筋注」発売時期の変更
サノフィは、国内製造委託先で一部の製造工程の確立と検証に想定以上の時間を要しているため、高用量インフルエンザワクチン「エフルエルダ」の日本での発売を、予定していた2026年秋から延期すると発表した。
これに伴い、75歳以上を対象とする2026～27年シーズンの定期接種への導入も見送られる。
日本以外の供給や、臨床試験で評価された安全性と有効性には影響しない。
エフルエルダは標準用量の4倍の抗原を含み、試験ではインフルエンザ発症を24.2％、肺炎・インフルエンザによる入院を8.8％多く減らした。
Sanofi has postponed Efluelda’s planned autumn 2026 launch in Japan because manufacturing-process validation is taking longer than expected. Its introduction into routine vaccination for adults aged 75 or older in the 2026–27 season has also been deferred. The delay does not change the vaccine’s evaluated safety or effectiveness.

2025年7月、サノフィ株式会社は、高用量インフルエンザHAワクチン「エフルエルダ®筋注」の日本における発売時期を、当初予定していた2025年秋頃から2026年秋頃へ延期すると発表した。
現在、詳細を調整中であり、確定次第改めて通知する。
製造および規制当局との手続きも新たなスケジュールに合わせて進行中である。

アジア諸国の新規抗菌薬の承認状況をまとめた報告。10か国が対象でCeftazidime/avibactamは10か国とも承認。Cefiderocolは3か国（日・台・韓）。Fidaxomicinは2か国（日・台）。Cefepime/enmetazobactamがインドのみで承認。Ceftaroline が7か国も承認は意外だった。

にわかには信じがたいが、まだピボキシル基を含む抗菌薬の処方で低カルニチン血症＋低血糖の事例が毎年こんなに起きている。死亡例まで出ている…。これは何とかしないとね、本当に。
