In 2006 we reported 44 patients with non-muscle-invasive micropapillary bladder cancer treated at MD Anderson. 27 got BCG. At a median 30 months, only 5 of them were disease-free with their bladder in place.
Twenty years on, we went back to the literature to ask whether that picture still holds. New meta-analysis in @EurUrolOncol

More evidence that NECTIN4 matters as a biomarker for EV+pembrolizumab! 🎯
Important independent validation from @MattGalsky and colleagues, now in European Urology @EUplatinum
In 322 patients receiving first-line EV+P for metastatic urothelial cancer, NECTIN4-high tumors had:
🔬 ORR 67.4% vs 41.9%
📈 OS 21.5 vs 14.2 months
➡️ adjusted OS HR 0.62
Importantly, NECTIN4 expression remained independently associated with response and OS in multivariable analyses — while NECTIN4 was not significantly associated with outcome in an independent pembrolizumab-monotherapy cohort!
➡️ true predictive biomarker!
Together with our data on membranous NECTIN4 expression, this adds to the evidence that the target itself carries clinically relevant information for a NECTIN4-directed ADC.
ADC biomarkers matter. Target abundance, localization, and heterogeneity may ultimately help us understand who derives the deepest benefit — and who may need an alternative ADC/ therapeutic strategy!

Important change in the 2026 @AmerUrological NMIBC Guidelines: all high-grade Ta tumors are now classified as high-risk.
This reduces a long-standing source of heterogeneity in the intermediate-risk group and should improve risk-appropriate treatment for patients, while creating cleaner, more meaningful populations for clinical trials.
Great to see the evidence translate into practice. @IBCG_BladderCA @UrogerliMD @pjhensley11 @AmirHorowitz @mouwlab @spsutkaMD @shilpaonc @AndreaNecchi @LAUrology_NL @karima_oualla @PGrivasMDPhD @marigfern @SpiessPhilippe @UroCancerMD
Time to revisit something I’ve been saying for years: TaHG bladder cancer is not intermediate risk. The clinical outcomes, molecular biology ... all point in the same direction.
Yet some frameworks allow < 3cm TaHG tumors to be downgraded into “intermediate risk” alongside low-grade disease.
That classification has real consequences for patients:
• undertreatment
• less rigorous surveillance
• in some studies, these patients are even placed on observation as a supposedly appropriate “control arm.”
TaHG is high risk. 🧵 @BladderCancerUS @WorldBladderCan @IBCG_BladderCA

Looking forward to welcoming colleagues, friends, patient advocates, regulators, and industry partners to Houston this week for #IBCG26.
The strength of IBCG has always been honest discussion, respectful disagreement, and a shared commitment to improving bladder cancer care.
Excited for two days of focused work together.

20 years ago, IBCG started with a small group around a table.
Later this month, friends and colleagues from around the world gather in Houston for #IBCG26. Two days around the table tackling some of the hardest questions in #BladderCancer, debating the evidence and challenging each other on where we go next.
Looking forward to seeing you all!

Proud to chair the 5th annual IBCG Bladder Cancer Forum at #AUA26 alongside an extraordinary group of global colleagues, thought leaders, and patient advocates committed to advancing care for patients with bladder cancer.
What began five years ago as a vision for open, high-level, multidisciplinary dialogue has grown into one of the most engaging and collegial forums at the meeting. This year’s debates and discussions once again reflected the passion, expertise, and shared purpose of this community.
My sincere thanks to all of the speakers, moderators, panelists, advocates, and attendees who made the session such a success.
