The compassion bag. We take plenty of equipment to the scene - compassion is perhaps the most important thing we bring…..@Kershberg @ehaatClinical Proud to say that I think we have some of the most compassionate clinicians in the world attend to our patients @EHAAT_

ECPR improves outcomes in refractory OHCA, but most patients never reach an ECMO center in time.
📍New systems are expanding access by initiating ECPR in the prehospital 🚑🚁 or non-ECMO center setting.
But what happens after cannulation?

Arterial cannula for percutaneous femoral VA #ECMO: relationship between cannula size, predicted arterial size & vascular complications
🧮 median predicted CFA diameter 5.4mm, 1.9 Fr lower vs chosen cannula
⚖️ significant discrepancy = higher complications
Estimated CFA diameter should be considered at cannulation, as well as target blood flow to support metabolic demands. Here a cannula sizing chart to guide selection @asaiojournal

🚨 In just 2.5 months since launch, RescQ has registered over 100 bystanders involved in Out-of-Hospital Cardiac Arrest (OHCA) events.
We're proud to be the first UK (at least!) to document the bystander denominator & provide with much needed support.🧵

I was highly skeptical that there was a benefit to recognising and treating occult VF diagnosed with POCUS versus traditional electrocardiographic (ecg)VF
This has made me revise my preconceptions.
There might be a benefit to diagnosing occult VF with US and treating it as such
Are you missing occult VF ( VF not recognised on ECG but visible on echo) in cardiac arrests masquerading as asystole OR even PEA? Check out the Reason 3 cardiac arrest trial and don’t forget to diagnose, defibrillate, and even consider ECPR in these pts.doi.org/10.1016/j.annemergme…

Long-term outcome of #ECPR vs cCPR in #OHCA
🇦🇺🇳🇿@EXCELoutcomes
🪦 28% ECPR/12% CCPR OHCA survivors at 12 months
⚖️ ECPR cohort younger, more likely to receive bystander CPR
⏱️ median arrest to ROSC/ECMO time longer in ECPR, 61 vs 6'
🧠 majority of ECPR survivors did not have significant cognitive disability at 12 months, but increased motor disability likely due to ECMO complications
Despite significant baseline differences, and prolonged duration of CA in ECPR patients, no differences in HRQoL at 12 months or return to work in survivors.

I don’t what more we can do… 80 patients with pre hospital arterial cannulation and intra arrest invasive BP measurements and it doesn’t even feature in the guidelines.
Meanwhile most services are just getting on with implementing the capability
Rob Major retweeted
Great point Chris!
This has been in other guidelines for adults for years, and can now be operationalised and delivered, even intra-arrest.
The A in ALS really doesn’t seem to stand for advanced, algorithmic life support would be more accurate

It is disappointing that this isn’t at least being considered for the adult population, given the recent work in the U.K. and overseas looking at AoDBP as a resuscitation target….
