Thanks again, @nickwallis. It is so important that @thetimes continues to pursue this story. It has long been my view, too, that this is the greatest medical scandal of our generation. For too long, some gender services adopted an ideologically driven model of care that paid insufficient attention to comorbidities, family trauma and the wider psychological difficulties underlying a young person’s distress. In too many cases, there was inadequate exploration of why a young person had come to experience their body as the problem, including the possibility of profound self-hatred. Some detransitioners have described how mental health practitioners failed adequately to understand or explore their experiences before they underwent medical treatment. Others report that surgeons did not clearly explain how surgically constructed genitalia would differ from naturally developed anatomy, including the possibility of continuing complications and limitations in sexual function. These accounts raise serious questions about assessment and informed consent. Instead, vulnerable young people were placed on a medical pathway involving powerful drugs and, in some cases, irreversible surgery. The @rcpch_president subsequently confirmed what many of us had been arguing for years: that these intrusive interventions, some with irreversible consequences, lacked an adequate evidence base. Medical and political authorities also failed for years to confront what was happening. The most basic principles of responsible clinical care, including careful assessment, curiosity, caution and first do no harm, were subordinated to an approach that treated the belief that medical transition was the answer as something to be affirmed, rather than understood and thought about before any potentially irreversible and damaging action was taken. That failure may have left some patients with lifelong consequences. It is shameful. #CassReview #PatientSafety #FirstDoNoHarm #GenderMedicine

Every medic, whatever their role, should read this letter.
Open Letter to Scrub Nurses Assisting in Gender-Affirming Surgeries in the United Kingdom. This letter is intended for scrub nurses and medical professionals. It contains graphic descriptions of highly controversial surgeries currently performed by some surgeons in the UK. It is not intended for members of the public or health care professionals who may find such content distressing. You will find it hard reading, sometimes we must face reality. If facing the truth about what is happening is difficult, consider that your discomfort pales next to the plight of the victims who are misled into giving consent for these operations. Spare a thought also for their families and next of kin, who must witness the consequences of this pseudoscience. "All tyranny needs to gain a foothold is for people of good conscience to remain silent." Dear Nursing Colleagues, I am writing to you as a fellow healthcare professional with deep respect for the critical work you do in the operating theatre. You have the hands that pass the instruments, the eyes that watch every move, and the presence that helps keep surgery safe. But in the context of gender-affirming surgeries—procedures such as vaginoplasty, phalloplasty, penectomy, bilateral orchiectomy, vulvoplasty, and bilateral mastectomy—your role goes beyond technical support. You are a key participant in these ethically troubling procedures that demand urgent scrutiny. Considering the recent UK Supreme Court ruling confirming that sex is a biological reality, the very foundation on which these surgeries are justified appears to be collapsing. The belief that sex can be reassigned by surgery is a medical fiction. There are three widespread deceptions involved in these procedures: 1.That sex can be changed or reassigned using hormones and surgery. 2.Those procedures such as vaginoplasty, vulvoplasty, and phalloplasty create vaginas, vulvas, or penises. 3.That removing the healthy breasts of a young woman somehow makes her chest a “male chest.” None of these claims are medically or biologically true. Two commonly held misconceptions — even within the medical profession — are that sex is assigned at birth and that a person can be born in the wrong body. These unfounded beliefs have given rise to a branch of pseudo medicine that underpins so-called gender reassignment surgeries. Biological sex is determined at conception and becomes clearly identifiable by the seventh week of intrauterine development. There is no scientific evidence to support the notion that anyone can be born in the "wrong" body. At birth, sex is simply registered, not assigned. Despite this reality, the language used during the consent process, in patient information leaflets, and even within the operating theatre often continues to imply otherwise. If patients are not fully informed—particularly about what these surgeries can and cannot achieve—then their consent cannot be considered truly informed and will be legally and ethically invalid. Terms like "vaginoplasty," "phalloplasty," and "vulvoplasty" suggest that a vagina, penis, or vulva is being created. In truth, this is profoundly misleading. You are expected to support these operations without question—even when they involve the removal of healthy breasts from adolescent girls, the amputation of normal penises and testicles from young men, or the construction of skin and fat flaps that are misleadingly labelled as “vaginas” or “penises.” You are expected to pass the scalpel into the hands of a surgeon who will first strip and de-skin the penis from the tip to the base, then proceed to disassemble the cylindrical structures of the hapless penis and then proceed to incise and amputate at the base of a perfectly normal penis. They then incise the scrotum, gain access to both the testicles (they have no escape now), transfix, ligate and cut through the cord structures and remove both the testicles from the physically normal adult young Man. Witnessing this in silence, you are to receive the severed organs into a kidney dish—alongside the bloodied blade. Afterward, you may be expected to dispose of them— discarding normal penises, testicles, and breasts from physically healthy individuals who are suffering from psychological or psychosocial distress (every human being goes through some form of distress in their lifetime which needs just psychological support). Your professional instincts may be screaming that something is profoundly wrong. You may be told this is "gender-affirming care." You may be instructed not to question. But medicine is not a ritual of obedience. It is a practice of ethics, science, and humanity. If your training, your conscience, or your professional experience is troubled by what is being done—listen to that voice. You are not alone. These surgeries are irreversible. They remove organs that are not diseased. They mutilate bodies that are anatomically and functionally normal. You are not just a pair of gloved hands. You are a thinking, feeling professional with the right—and the responsibility—to question what you are being asked to do. Silence does not equal consent. And history has not been kind to those who said, “I was just doing my job,” when the job involved harm to patients. I urge you: do not turn away from your discomfort. Reflect on it. Speak about it. Document it. You may be the only voice in the operating room who still sees the patient as a human being, rather than a set of parts to be rearranged or removed. Your voice matters. Use it. Let me be clear: Gender dysphoria is a psychological condition, listed in the DSM-5 psychiatric manual. It is not listed in any reputable surgical textbook as an indication for radical, irreversible surgery. The amputation of healthy organs like penis and/or testicles in response to a psychological diagnosis would not be accepted in any other context in modern medicine. The Medical Director of my health board in Wales—a respected senior physician—has noted that the primary risk of not proceeding with surgery is psychological distress. Traditionally, and always, such distress is managed by mental health professionals and general practitioners, rather than through irreversible surgical procedures. Furthermore, the Cass Review highlights that many of these interventions lack a robust evidence base and have not been subject to long-term, high-quality research. I know that scrub nurses are not the decision-makers in theatre. But you are not without power. You can raise questions, escalate concerns, and refuse to participate in procedures that violate your ethical judgement. Silence and complicity, even with good intentions, can no longer be defended as professionalism. I urge you to reflect on the following: These surgeries permanently destroy fertility, sexual function, and normal anatomy in otherwise healthy young people. You are helping to remove normal organs—organs that are later incinerated. With growing awareness, public scrutiny, and court rulings upholding biological reality, these procedures may soon face legal challenges, possibly under grievous bodily harm (GBH) statutes. If it is later determined that the psychological evaluations were inadequate, or that consent was based on distortion, omission, or deception then your participation in these procedures—however well-intentioned—may be reevaluated in a very different light. Therefore, I urge scrub nurses to: Seek guidance from your professional bodies and ask whether it is ethical and legal to assist in operations that rely on misinformation to secure consent. Escalate your concerns through hospital safeguarding teams or whistleblowing procedures, especially when young adults or vulnerable patients are involved. Request formal clarification from the Nursing and Midwifery Council (NMC) regarding your ethical obligations in surgeries where the consent may be deemed invalid due to the deception involved. None of us entered medicine or nursing to harm healthy human beings. Yet these surgeries, I believe, cause harm—lifelong, irreversible harm—under a veil of affirmation and false hope. In this situation, ethical courage begins not with confrontation, but with asking honest questions. You are not alone in your doubts. Many professionals— surgeons, anaesthetists, nurses, GPs—are beginning to speak out. I invite you to join them. With respect and urgency, Dr Joseph Chrysostom MBBS, MS (Gen Surg), FRCSEd ATLS Instructor, Royal College of Surgeons of England GMC-5199143.

Transgender surgery is like the abandoned practice of frontal lobotomy, according to prominent US plastic surgeon Dr Scot Glasberg. He suggested the argument for stopping under-19 trans surgery was the same for lobotomies—“there was no evidence to show that [these interventions] were of any benefit, and we were harming people in the process”. He also cited the recent comprehensive Finnish registry study which found that the psychiatric needs of young people increased after gender medical treatment. Dr Glasberg said this study was important because it highlighted the risk that “it actually could be worse for these children” after gender transition. “I thought, this is pretty telling.” Dr Glasberg is a past president of the American Society of Plastic Surgeons which earlier this year became the first major US medical association to adopt a policy advising against the gender medicalisation of adolescents. He was speaking at a London conference on “Rethinking Youth Gender Medicine” hosted by the Society for Evidence-based Gender Medicine and the Clinical Advisory Network on Sex and Gender. Cuts through the ideological flannel and gets right to the heart of the issue. This is what every politician needs to understand.
Brilliant article on why the "conversion practices bill" could become Labour's section 28.

Must read. If you’re not already aware, you really should be.
1/ The 2012 Tavistock patient survey showed 80-90% of the teenagers there were same sex attracted. Something like 30% were on the autism spectrum. Staff at the Tavistock had a dark joke "soon there will be no gay people left". Senior leadership resigned because Mermaids ran riot

The BBC’s failure to cover the GIDS scandal has been a serious dereliction of its public duty. After resigning as a governor of the Tavistock, I was involved behind the scenes in the 2019 Panorama programme on GIDS. The producer warned me I might be disappointed the programme would expose only one part of the scandal, not the whole story. That warning told me everything I needed to know about the BBC’s problem with impartiality on this issue. When Sue Evans and Keira Bell won the first judicial review, there was no serious follow-up from the BBC News, Woman’s Hour, or other major outlets. Nor was there adequate coverage of one of the most important findings of the Cass Review: the poor quality of the evidence base behind medical interventions for children and adolescents with gender distress. Instead, the BBC repeatedly broadcast positive stories about transition while failing to investigate the harms, uncertainties, and institutional failures surrounding this field. Sue and I have seen some of the casualties. Parents have been badly let down by professional bodies that lacked the courage or independence to challenge the affirmative model none of which received the scrutiny a public broadcaster exists to provide. With honourable exceptions in Newsnight and the Today programme, the BBC has failed young people, failed parents, and failed in its most basic public duty.

This is a truly remarkable article. Firstly, it’s the voice of a courageous detransitioner @redrobin9000, who speaks candidly about her experiences. Secondly, it’s the foremost article in the publication. It deserves to be widely read. Credit to Phillippa, to @nickwallis and to the editor of YOU @JackieAnnesley. dailymail.com/lifestyle/arti…

The lack of evidence for mastectomy in response to gender dysphoria applies equally to adults, yet the NHS makes over 1000 referrals a year for these surgeries (link in next tweet). Removing a young woman's breasts in response to psychological distress is a dereliction of the clinician's duty of care to protect vulnerable patients from harm. The question is, what will it take for the NHS to alter course?
Replying to @segm_ebm
About 1,000+ mastectomies for gender-dysphoric minors are performed each year in the U.S, most done by plastic surgeons. The ASPS statement suggests this practice will be upended. But the analysis that ASPS offers for the rest of the transition pathway signals far more profound changes are likely to come. The ASPS asserts that all steps in youth transition—from social transition and puberty blockers to cross-sex hormones and surgery— are interconnected; that all share similarly problematic risk–benefit profiles; and the risk compounds with each subsequent step. This puts other medical associations on notice. At the very least, they have to review and adjust their own positions—or justify why they continue to stand by a practice that is not evidence-based and is profoundly ethically challenged. /2 segm.org/ASPS-position-state…

I've signed. It's over 60k signatures now. Puberty blockers are part of a treatment pathway that is expressly aimed at turning gender-distressed children into adult eunuchs. The trial is an abomination.
🚨We’re half-way there!🚨 50,000 signatures in just two days is huge. This is now one of the fastest growing petitions of all time. We need to get to 100,000 as soon as possible to force a parliamentary debate on this. Please continue to sign and share: petition.parliament.uk/petit…

As 2026 begins, I urge everyone to focus on exposing the utterly unethical and essentially homophobic basis for the proposed puberty blockers trial and on doing all we can to stop it. The PATHWAYS trial is designed for children with “gender incongruence”. This silly jargon syndrome generally describes a girl or boy who is unhappy with sex stereotypes. Most of those concerned are LGB teens. LGB teens are not sick! Nor are other “gender non-conforming” children. Giving them drugs known to cause harm, which will place them on a path to lifelong medicalization, as well as leading to sterility and numerous health risks, and in many cases to impaired sexual function as adults, is inexcusable. That is why the drugs were rightly banned in the first place. It is time to stand up to the unscrupulous and/or deluded activists who are pushing this trial! Wes Streeting must be urged to prioritise the Data Linkage Study on the 2000 children who have already taken these drugs. Let’s look at the information already held before we risk the health of any more.

It looks like rolling recruitment with no endpoint. After two years children can continue on blockers and then cross-sex hormones, outside trial conditions. The trial is just the entry conditions onto the medical Transition Pathway. #stopthepbtrial #StopTheTrial

1/ This is an important demonstration. GMN will be joining the LGB Alliance and others to make the point that Gay teens are not sick. They do not need medical correction. Puberty blockers (as the Secretary of State said when banning them) are dangerous and experimenting is wrong.
Join us outside the Dept. of Health at 39 Victoria St in London to oppose a drugs' trial on majority LGB young people. Read more about our objections here. lgballiance.org.uk/gay-teens… #WriteToWes
