JNJ-6143, first-in-class oral AR-BRD4 RIPTAC, phase 1 mCRPC: a wide window between efficacy and safety. n=105, doses 12.5 to 150 mg. 📈 50 mg daily: PSA50 77%, PSA90 48%, confirmed ORR 69%, after a median 4 prior lines 📊 PSA declines seen from 12.5 to 150 mg. 66% of all patients still on drug 🔍 At 50 mg: 58% any-grade TRAEs, no serious TRAEs. Median DOR 11.0 months, CI 5.9 to NE Activity across a 12-fold dose range. Phase 2 is open at 50 mg. The VA leading the way. Congratulations to all of the physicians for overcoming barriers and implementing best practices.
In the Veterans Affairs health system, active surveillance or watchful waiting for favorable-risk #ProstateCancer increased from 27% to 93% for low-risk disease and from 14% to 61% for favorable intermediate-risk disease. ja.ma/4cnGmcs

Dr Kyle was a giant of Medicine in a way that few of us can ever aspire to. His transformational impact on hematology was in no way fortuitous or accidental, but the direct result of his tireless work ethic and understated brilliance. He was also the consummate gentleman and an extraordinary mentor. I consider it one of the great blessings of my career that my first papers as an intern were with Dr Kyle. I still have the first drafts almost entirely covered by his red pen corrections, each one of which was precise, illuminating, and irrefutable. God Bless you Dr Kyle. Thank you for your generosity and kindness.
We are deeply saddened by the passing of Robert A. Kyle, MD, founder and first president of the International Myeloma Society. Dr. Kyle’s research changed how the medical community understands multiple myeloma and the conditions that can precede it. He was also a teacher and mentor whose influence can be seen in generations of clinicians and scientists working in the field today. From helping convene the first International Myeloma Workshop in 1987 to founding IMS and serving as its first president, Dr. Kyle believed that progress depended on experts working together across borders, with patients always at the center. That commitment to science, collaboration and patient care remains at the heart of IMS. We extend our deepest condolences to Dr. Kyle’s family, friends, colleagues and the many patients whose lives he touched. myelomasociety.org/rememberi…

A special moment at our USPCC conference when the FDA approval for Pluvicto in APMS occurred mid conference. Congratulation to the study leaders- our friends and mentors @sartor_oliver @DrScottTagawa and Michael Morris, and congratulations to @novartis and to the patients and staff who made this study a success.

Indeed. That’s why the surgery without adt sub study is so important for the readout against the current surgical soc. Also, the study patients largely received salvage radiation on progression so the secondary endpoints reflect that. From the article supplementary data @nejm
Standard of care if you are going to operate is surgery then salvage if needed. This was not included in the trial so hard to say this is a postive trial for neoadjuvant therapy where this has not been tested and all previous trials negative.

Key takeaways for rucaparib in BRCA mutated mCRPC: - Drug discontinuation rate in the rucaparib arm of 14% vs 22% in the ARPI or Doce control arm - median onset of cytopenias is over 2 months, meaning plenty of time to see it coming and manage preventatively - Median Rucaparib dose intensity over 98%, meaning full dose was given for over 98% of total doses administered
Safety of rucaparib vs docetaxel or second-generation androgen receptor pathway inhibitor therapy for patients with #mCRPC and a BRCA mutation in #TRITON3. Presentation by @AlanBryce9 @cityofhope. #AUA26 written coverage by @RKSayyid @UAUrology > bit.ly/3ROWcFh

25% discordance between PSA and radiographic progression is also what we saw in PREVAIL and CHAARTED so the finding is clearly consistent. If it feels high that is likely because your real world definition for PSA progression is more liberal. nature.com/articles/pcan2016… sciencedirect.com/science/ar…
I did struggle with this despite being kindly involved in some of the studies. 25% feels high but anecdote versus data never a good thing. @Prof_IanD @ChrisSweens1 guide me. No PSA progression but LDH, LFTs, FBC, Albumin. Would they miss this? It’s clearly more than I feel clinically which would be closer to 10% @Silke_Gillessen @alison_tree @AdamSharpMedOnc @AlanBryce9
