The placebo response in #NASH, simply explained
@AGA_CGH tinyurl.com/dkvfxhd4
1⃣ Sampling variability explains the majority of the placebo response
2⃣Fibrosis progression in trials slower than in observational studies
3⃣Is liver biopsy a reliable endpoint? #LiverTwitter

Conference abstracts serve a purpose for students and trainees
There is the experience of doing the work
It gives people a chance to be part of the community, to meet experts and future collaborators and colleagues
@basl_events the poster round is the BEST part of the meeting
In defense of conference abstracts
I went to @DDWMeeting in 2009 as a medical student. I worked HARD to produce an abstract. I learned a TON. I was in AWE of all the talks from people whose papers I read. An EXPERT came by my poster and destroyed it. I will NEVER forget

Great fun think about this for @JHepatology
Need to think about whole patient benefit of treatment for individuals with #MASLD and other metabolic multi morbidity
Many patients with MASLD will receive effective treatment before hepatology involvement
Brilliant editorial by @IanARowe: A “typical" (T2DM, HTA, BMI 34, DLP) at-risk MASH patient faces ~12% 10yr CV risk but only 1-3% liver-event risk. This gap shows management should prioritise weight loss and metabolic control before liver-directed therapy.journal-of-hepatology.eu/art…
Privileged to be part of this wide-ranging discussion #EASLStudio at the #EASLCongress
We explored lots of the important issues around screening for liver disease and why it is critical that we build the evidence base to support it
Free to watch back, it's worth the time!
Watch #EASLStudio to find out how are projects like @LIVERAiM are aiming to change the game when it comes to diagnosing #LiverDiseases early

This was very much a team effort!

Much value in reproducing work in this context, highlighting the role of #alcohol in driving liver disease

Important data in #MASH but probably not the game changer some think
75% of patients had a potential indication for SEMA, BMI>30 or T2DM
GLP1RA is a foundational treatment for patients with metabolic multimorbidity
If initiated for those reasons, MASH indication not needed
Semaglutide improved inflammation and fibrosis in people with MASH / NASH
1. Generalizable gem: 10% body weight loss is the target to reverse fibrosis
2. Monkey on the back: inflammation endpoints with huge placebo effect

So much to say on #ESSENCE
Placebo response = sampling variability, see cghjournal.org/article/S1542…
Also, improvements in steatosis at least are mediated through weight loss, see
jhep-reports.eu/article/S258…
@ebtapper
Replying to @ebtapper @enndriitk
The placebo gp had mean 2% weight loss suggesting that you don’t need to lose that much weight to have a favourable reduction in steatohepatitis (1/3 of the placebo go had a reduction in steatohepatitis). Whilst GLPs are game changers, lifestyle modification makes a difference.
This should be a fun discussion - there is no perfect scheme for allocation of livers...
