PD(L)1-VEGF bispecifics are already beating PD1 inhibition alone in trials, so what's next? This trial (BioNTech, DualityBio, BMS) adds a new B7-H3 ADC to a PDL1-VEGF bispecific, with potential synergy: 1) VEGF blockade: normalizes tumor vasculature, improves ADC delivery and T cell infiltration. 2) B7-H3 ADC: kills cancer cells, triggers immunogenic inflammation. 3) PDL1 blockade: releases immune suppression, allows T cells to exploit immune response induced by ADC. Confirmed ORR up to 71% -- in immune-cold platinum-resistant ovarian cancer. The broader, ongoing phase 2 includes colorectal cancer, pancreatic cancer and other tumor types.

From the ESMO abstracts: Interesting results for Pumitamig in 2L neuroendocrine neoplasms, especially when compared to the older bevacizumab trial, since Pumitamig is showing an ORR *and* OS benefit here:

Has anyone tested combining a PDL1xVEGF bispecific with a PD1 monoclonal? - The idea being that the bispecific would anchor the VEGF inhibition to the TME, whereas the PD1 mono would reinvigorate T-cells across the body? Anyboy done this expertiment? @AppleHelix @DrSamuelBHume

Love this from @gline! “biotech is a wood chipper that like is built to like ingest souls and shred them. It's just like such a hard business that like someone comes and they're like, oh, but I've built a computer and I'm going to push a button and all of that goes away and it just works. Like that is probably not the way that's going to go.”
If you care about biotech and drug development watch this interview with Matt Gline, the CEO of Roivant, a biotech firm that is now worth $24 billion. He talks about why he thinks predictions about AI from people like Anthropic’s Dario Amodei are overly optimistic. “If Dario is right and I'm wrong, and he cures every disease in five years, man, are we all the winners from that outcome,” Gline said. “I think if you work in this industry, it is very hard not to be skeptical of any of those claims. Because, and I've said this in other settings recently too, biotech is a wood chipper that like is built to like ingest souls and shred them. It's just like such a hard business that like someone comes and they're like, oh, but I've built a computer and I'm going to push a button and all of that goes away and it just works. Like that is probably not the way that's going to go.”

Here's a cool new drug that just dosed its first patient in China. It targets the cancer antigen, B7-H3, and 'desialylates' - removes surface sialic acids that suppress immune responses. In experiments, this activates tumor killing, mainly by NK cells and macrophages. The trial is in China, run by Shanghai Henlius - the tech itself originated from the US biotech, Palleon Pharma. This desialylation mechanism is independent of PD1: in other words, a potentially new cancer immunotherapy.
By my count, China has cleared 160 novel drugs for first-in-human clinical trials this year — so far. One of them is a potential treatment for sepsis, a condition associated with ~20% of all deaths globally. The drug, made by Pyrotech Therapeutics, is the first to enter the clinic that blocks formation of the gasdermin D pore (that triggers pyroptosis - inflammatory cell death - in sepsis). That’s just one of many — we looked at all of them here: breakingground.substack.com/…

Very interesting, Roche will initiate a phase III glycemic control head-to-head study of Enicepatide versus Tirzepatide in patients with T2D. They released some new data yesterday for the BLA1c > 8.5% subgroup. @JCanNuSH

Anyone really surprised by this?
Anthropic's discovery of array-associated reverse transcriptases? Turns out it was already done by a Danish scientist. He used Claude for administrative tasks, but not in an agentic manner for the discovery. He already filed a patent and gave talks on it nytimes.com/2026/09/27/scien…

Was surprised by the near convergance of the Ivo and Pembo OS curves around month 10 in Harmoni-2. Looks like the explanation for this is that non-responders in the Pembro arm switched relatively quickly to pembro+chemo combo. This makes the results even more impressive actually.

Would be very interesting what Julie Brahmer has to say about this subgroup analysis here with Ivo working *better* in older patients:

Truck test ✅ A particularly neat feature of this trial: it compared an unconjugated topo1 inhibitor (topotecan) vs a conjugated topo1 inhibitor, demonstrating the impact of the ADC delivery system 🎯
Presented at #WCLC26: Among patients with relapsed small-cell lung cancer after chemotherapy, treatment with tambotatug pelitecan led to longer survival than treatment with topotecan, with fewer adverse events of grade 3 or higher. Full phase 3 TAISHAN-302 trial results: nej.md/4xKtvcF @IASLC

We now have TWO studies showing B7-H3 ADCs vastly superior to standard topotecan for relapsed SCLC: tam-peli and ris-rez. These studies were different - different agents, different baseline demographics of pts, etc - but the outcomes were similar, control arms performed similarly. These trials reinforce each other. Looking forward to global studies and to first-line studies. The ADC era soundly here for SCLC and these will replace chemotherapy across lines, in my opinion. #WCLC26
