Here is a new paper for the lipid nerds from the Balance and ESSENCE trials: Lp(a) is genetically determined—but can change due to metabolic factors. In genetically confirmed FCS, Lp(a) is markedly suppressed, especially in those with small apo(a) isoforms genetically programmed for high Lp(a). Why? Likely the absence of normal LPL-mediated lipolysis/remodeling in a chylomicron-dominant state, altering the apoB-100 particles available for Lp(a) assembly. And the OxPL story (@OxPL_apoB) is striking: OxPL-apo(a) falls disproportionately, with a ~70% lower OxPL-apo(a)/OxPL-apoB ratio—evidence that OxPL partitioning shifts away from Lp(a). jlr.org/article/S0022-2275(2…

Lp(a) carries biologically active oxidized phospholipids (OxPL). New research links its OxPL cargo to platelet hyperactivation through CD36, connecting Lp(a) biology to thrombosis and highlighting the importance of what the particle carries. Lp(a) may be more involved in platelet function that previously appreciated, see our review on this also. ncbi.nlm.nih.gov/pubmed/3793…

We have a new paper today that links Lp(a) with triglycerides in patients with moderate hypertriglyceridemia (TG 150-500). Using our new methods we developed @UCSDCardiology Lp(a) becomes TG- and cholesterol-enriched. ApoC-III inhibition reduced Lp(a)-TG and Lp(a)-C by ~60–70% without lowering Lp(a) particle number, revealing pharmacologically modifiable Lp(a) heterogeneity. We now need to see how Lp(a)-C and Lp(a)-TG correlate with CV outcomes. sciencedirect.com/science/ar…

My suggestion to everyone speculating is wait for the full results, not just of HORIZON but the other trials. Then send the preponderance of evidence to the jury to decide on the Lp(a) Hypothesis. We really need to know its (patho)physiological role- why does nearly every human have circulating Lp(a)? And why does only human Lp(a) carry OxPL?. We should be grateful so much investment is ongoing to find out what it means to humanity.

Lp(a)HORIZON topline results: pelacarsen lowered Lp(a) but did not reduce cardiovascular events, in a trial where background risk was exceptionally well controlled (mean LDL-C around 66 mg/dL).
novartis.com/news/media-rele…
There are many lessons here for future trials. Clinically, not all Lp(a) elevation is created equal. We need to know whether isoform differences drive risk and personalize management accordingly.
Lp(a) also does not act in isolation. As we argued in our European Heart Journal editorial "Two to Tango" (academic.oup.com/eurheartj/a…), it is the interplay among risk factors that ultimately leads to cardiovascular events. Many researchers have shown that controlling modifiable risk factors substantially lowers risk in patients with elevated Lp(a), so knowing your number remains a powerful driver of prevention, even without a targeted therapy.
This result changes nothing about the enormous burden of Lp(a) or the case for universal screening and public education. It sharpens the questions we must answer: who is at greatest risk, why, and how best to protect them. Preventive cardiology owes a great deal to Lp(a). It has unlocked the potential of prevention and CV risk modification. @hsbhatia @CMichaelGibson @MWilkinsonMD @DrMarthaGulati @DavidLBrownMD @kaulcsmc @RonBlankstein @Drroxmehran @DrEugeneYang @ProfKausikRay @VietHeartPA @drpablocorral @JohnKastelein @CBallantyneMD @Drroxmehran @escardio @ACCinTouch @hmkyale @ASPCardio @AnnMarieNavar @DrMichaelShapir @mdavidsonmd @EugeniaGianos @BudoffMd @MattRamsisMD @pnatarajanmd @EricTopol @EricAdler17 @society_eas
