2. Pip-tazo
Pip-tazo can inhibit renal clearance of methotrexate (MTX) and results in toxicity esp when high dose MTX is used (e.g. for hematological malignancy)

Please don't mix fruits and a whole lot of raw green and colored veggies into a blender and make yourself a detox juice if you have pre existing liver disease.
This is the second patient this week who seems to have developed kidney injury after daily consumption of home made fruit+veggie juice.
A blend of gooseberries, beetroot, spinach & green leaves along with a host of citrus fruits is a new fad thats developing after WhatsApp and YouTube "doctors" have been promoting then as "liver health" options.
Please don't do it. It can cause OXALATE KIDNEY INJURY and the kidneys take a LONG time to recover.

💡Smoking is associated with higher baseline WBC counts and with leukocytosis
💡These changes are reversible
💡Activation of alveolar macrophages and a general inflammatory response are largely responsible for the effect on WBCs

In real life we are not trying to decide whether to use Pomalidomide or belantamab. But rather what to do when pomalidomide based regimens stop working. A response rate and response duration seen with belantamab would be a good option to have. But we won’t. At least for a while.

Past medical history:
Wrong:
- AF
- DM
Right:
- Persistent AF managed w/rate control/anticoag, CHA2DS2-VASc 3
- Type 2 DM, Hgb A1c 7.5% 7/20, no end-organ dz
The PMH should provide descriptions of dz severity. Don't list- stratify!

I am a medical oncologist, and a frequent part of my job is giving people bad news. Due to the pandemic I have been doing almost all of my clinic visits by phone or video the past three weeks, and I have some reflections on giving bad news remotely 1/9

Most head and neck cancers (73% in the U.S.) are now related to human papillomavirus infection rather than tobacco and alcohol. Primary cancers are largely squamous-cell cancers, but combined treatment is resulting in long-term survival for most patients. nej.md/36j57Cs

Different pathogenesis. Intra-luminal platelet aggregation is seen in TTP, not aHUS. But brain can be affected in HUS, largely global ischemia rather than Focal lesions.

Evergreen from @JAMAOnc: “Using the battle metaphor implies that if a patient fights hard enough, smart enough, and/or long enough, he or she will be able to win the war.”
This is simply not true for some & I’ll be damned to imply they are losers jamanetwork.com/journals/jam…

Marvelling at nature. We have 20-30 trillion red blood cells - each contain 270 million hemoglobins - that is about 6 or 7 x 10e21 hemoglobins. That is six sextillion! On the order of grains of sand on all the beaches in the world or stars in the universe. #hematology #phodocs – at Detroit, MI

The paradigm is diagnose then treat.
There are many cases where we treat first and diagnose later.
- pulmonary edema
- sepsis
- Vfib
- respiratory failure
Always notice when you are doing it backwards and justify this upside down medicine in your note.
