Response to IVCS in ASUC is dynamic.
Day 3 partial response ≠ treatment failure — some patients continue to improve through day 5–7.
This study puts data behind what many of us have been observing clinically..

You are replaceable at work, but you are NOT replaceable at home. A beautiful and poignant reflection that applies to all clinicians.

🚨 Do you really need a high-end ultrasound machine for IUS?
Our HUMID study: (eClinicalMedicine @eClinicalMed ):
📊 Prospective paired study (n=450)
🔬 Benchmarked against tandem ileocolonoscopy
✅ Mid-end = High-end performance in ileocolonic IBD
👉 Enables scaling in low-resource settings

Is the rectum still a “blind spot” in IUS? 🤔
For many years, assessment of the rectum in IBD relied almost entirely on colonoscopy.
But with Transperineal Ultrasound (TPUS), this paradigm is changing. 🔍
A simple ultrasound probe placed gently over the perineal region allows non-invasive visualization of the rectum.
🚫 No scope
🚫 No bowel preparation
🚫 No sedation
📡 Microconvex vs 📶 Linear probe — our study explores whether commonly available linear probes may be sufficient for TPUS. Accuracy not different but combination📡 + 📶better;📡 has technical advantage due to small 👣
Our work from 🇮🇳 @AIGHospitals contributes to the TRIUMPH-UC study — aiming to standardize TPUS and integrate it into routine IBD care.
Grateful to Dr Mateen sir , @AnuradhaSekara1 and her pathology team, the great duo Dr Kanapuram Pooja & Dr. Syed Misba Naaz and other colleagues
🙏@DrDNReddy sir for the guidance
A small technical step…
but hopefully a TRIUMPH for UC patients. 🏆

❓How fast should we taper steroids in Ulcerative Colitis?
Longer taper (10 weeks) better than a shorter course (6 weeks)
- Balancing relapse vrs adverse effects
- Setting of 5-ASA + Thiopurine use
- Probably not applicable to advanced therapy
