💡 JAMA Insights: #CeliacDisease is an autoimmune disease triggered by dietary gluten and affects at least 1% of the global population. Diagnosis should be performed while patients are consuming gluten, with tissue transglutaminase IgA testing and total serum IgA used as first-line screening to support accurate detection. A serology-based diagnosis may be appropriate in selected children and adults with tissue transglutaminase IgA levels ≥10 the upper limit of normal. Biopsy remains important for patients with IgA deficiency, alarm symptoms, certain autoimmune disorders, or certain chromosomal disorders. ja.ma/3VIdc2h

What happens in the rectal mucosa before — and potentially as part of — perianal fistulising Crohn’s disease? Gudiño et al. present “TL1A-activated T cells remodel the rectal mucosa in patients with Crohn’s disease with perianal fistulising disease” via bit.ly/3V1eEMH Using single-cell RNA sequencing of rectal biopsies from patients with Crohn’s disease with and without perianal fistulising disease, together with functional studies in T cells, tissue explants, fibroblasts and epithelial cultures, this study identifies a distinct molecular signature associated with fistulising disease. The rectal mucosa of patients with perianal fistulising disease showed cellular and transcriptional alterations that were independent of luminal inflammation. The researchers implicate TL1A activation in CD4+ T cells and identify lymphotoxin β and IL-22 as downstream mediators affecting fibroblast and epithelial compartments. Importantly, the TL1A–LTα1β2/IL-22 pathway operates independently of TNF signalling, suggesting that these molecular changes may persist even in the context of anti-TNF therapy. The findings provide new insight into the mechanisms that may predispose to perianal fistulising disease and support further investigation of TL1A-directed approaches in this challenging complication of Crohn’s disease. #CrohnsDisease #PerianalDisease #TL1A

Today is packed on the IBD track - one of the most important conference days for IBD I can remember. You can't see everything unless you clone yourself ** ... so head to UEG forum for 10am • VERDICT primary outcome * • STRIDE-III * • FUZION 48w * • ABBV-466 starring Vip, Axel, Ailsa and Remo * first look at all of these (still under embargo) ** if the cloning worked go to D3 for obefazimod ph3 maintenance data (already in the public domain) THEN ... 1130am to A2 for the 5y Profile data (or just read the paper @LancetGastroHep ) and 1150am to A1 for IBD nutrition matters 230pm Lessons from large IBD real-world cohorts (A1) 4pm Teaching an old dog new tricks: New implementations of current IBD drugs (A1) including AFFIRM-UC (sc risa induction) and DIRECT-CD (sc ifx induction) and much more. Oh, there are a bunch of live poster sessions too! THEN ... come to the @my_ueg night and drink a beer with me.

📰 New review in the November issue of Frontline Gastroenterology Abuquteish et al review lower GI toxicity from immune checkpoint inhibitors and targeted cancer therapies, covering clinical, endoscopic and histological features. 🔑 Anti-CTLA-4 causes earlier and more severe colitis than anti-PD-1/PD-L1, and colitis was reported in about 25% of patients on combined ipilimumab and nivolumab. Rituximab was linked to a roughly sixfold increased risk of developing IBD. 🔗 doi.org/10.1136/flgastro-202… @BritSocGastro @dr_aditi_kumar @OTavabie @DrJMKennedy @DunnePdj @TrevorTabone @eathar_s @IrenePerezMD @KGananandan @zare_benjamin @dtleiberman
