New from the EAAA RAID team with @EastAngliAirAmb, @EHAAT_ and @Magpas_Charity, published open access in @JAMANetworkOpen: post-intubation hypotension after prehospital RSI in severe TBI is strongly associated with higher 30-day mortality.

I preliminarily have a MSc in advanced practice! A journey which started in 2019 amongst a cohort of critical care Paramedics as a ‘standard road Paramedic’ full of ambition. Throughout the COVID pandemic, full time work, and the birth/ fathering of my twins. I’m pretty proud!

Great to see this published in @Anaes_Journal
TLDR -
Stylets best for hyperangulated VL intubation
Dynamic bougies next best
Standard bougies worst
Effects more marked when difficulty increases
Manikin studies are often criticised, but may in some cases be the optimal design.
There is a weakness of most clinical studies: even in 'high risk' or 'predicted difficult' cohorts most (>80%) intubations are simple so don't stress test the technique. This creates a false reassurance in results.
We used a specific difficult intubation manikin from @LimbsandThings1
It enabled us to set up a reproducible
- straight forward HAVL intubation
- genuinely difficult one (tricky 2b view)
So we really did stress the technique....
We studied
-Storz C-MAC Stylet (@KARLSTORZUK)
-Universal Stylet Bougie (@intersurgical)
-Total Control Introducer (@RunnelsSean)
-Steerable Tracheal Intubation Guide (@LateralMedical)
-Portex Single-Use Bougie
30 participants (familiar with D-blade use) intubated the manikin with each adjunct (after familiarisation, random order) in the easy and difficult set-up (again randomised)
Clear separation of performance
-Stylets best (excellent)
-Dynamic bougies next best (still good)
-Standard bougie worst (poor)
When the chips are down the separation of performance was increased
-stylet FPS close to 100%
-bougie performance close to 35%
Success rate, time, effort, VRS scores all support these findings.
Don't get fooled into thinking bougies work well with HAVL by studies that don't stress test the technique.
Don't get stuck with the wrong tool!
Well done to all the staff at @RUHAnaesthesia who did the study or participated & @VirtueOfNothing

We're investing in the Beech Band, a wearable device that aims to manage Parkinson's symptoms.
Designed by @plantsbloke, the device is showing promising signs of being able to manage symptoms including speech issues, anxiety and gait.

🤔Why does subarachnoid hemorrhage lead to deep/inverted "cerebral T-waves"?
This ECG finding is so dramatic. But as we will see, these patients often have normal hearts.
Why does an issue in the brain manifest on a test of the heart?

Ok. A few bits. Will add a context of intravenous sedation for long bone fracture manipulation as an example. 1. Even when the pre-sedation environment is tense, noisy, loud, etc., speak gently with the patient before you start. Allocate someone to continue to speak smoothly and

Welcome to #JanuAIRWAY2024. Every weekday we'll be bringing you Airway #FOAMed. Starting off with management of transgender airway -here's a #OnePager. It's a huge topic going beyond just the airway. Thanks to Drs @LukeFlower1 @drkamillak & Alice Humphreys for all their help! 1/7

Hopefully the last OSCEs of this MSc were completed today ✅ now it’s just the nervous wait for results 🤞🏼🤞🏼
Hoping to figure out what suction equipment other trusts use throughout the UK...
In SWAST we have the laderal Suction unit with yankaeur, 6.2mm internal diameter tubing, 10 + 16fg flexible suction, and the bulb type manual Suction...
What do you carry in your trust?
Anyone who works for @SECAmbulance @SECAmb_Research @SCAS999 @NIAS999 @Scotambservice @NEAmbulance that may be able to help with what suction equipment you have? 😊
Well, just submitted the 48 hour exam paper. When this was mentioned I didn't actually think I would be waking up at 08:30 and working pretty much continuously until 03:00 on two consecutive days... Questioning whether I made it harder than it needed to be 🥴 so glad it's done 🥳
