General neurology is the foundation of every neurology supra-specialty!🧠 This superb review in @GreenJournal by @SanRaffaeleMI is a great chance to brush up on cranial neuropathies, with practical algorithms and clinical pearls. This is what clinical reviews should be: clinically focused and practical, going beyond a narrative summary of the literature that AI could write! 👉🏼doi.org/10.1212/CPJ.00000000… #Neurology #NeuroTwitter

I am excited to share my review article, “Amyloid Neuropathies,” published in @ContinuumAAN. My journey caring for patients with amyloid neuropathies began 14 years ago at the Brazilian Amyloidosis Referral Center at my alma mater, @ufrj. Since moving to @MayoClinic, I’ve continued my clinical work and research in amyloidosis, which has given me a unique perspective on patient care across two very different settings. I’ve aimed to simplify this complex topic, including many clinical pearls and a diagnostic flow chart highlighting key red flags. I’m also proud to share that @LyellJ selected this article as one of the editor’s picks, making it freely available for the next three months. Don’t miss out—download it now! 😊 continuum.aan.com/doi/pdf/10… Lastly, I want to say it's such an honor to write for @ContinuumAAN, which was the most important neurology journal throughout my (long) neurology training journey. The opportunity to give back through this review was very meaningful to me. I hope you all enjoy it!

One unintended consequence of the rise of tele-neurohospitalists and shift-based neurology models is that no one clearly owns the patient’s care beyond the acute hospitalization. Too often it’s 5 days of IV steroids… and adios. I frequently see patients with MS, NMOSD, MOGAD, or autoimmune encephalitis discharged with, “Follow up with neuroimmunology ASAP.” No steroid taper (when appropriate), no plan for relapse prevention, and no baseline screening to facilitate long-term immunotherapy. Some return weeks later with another preventable relapse. Hospitalization should be more than treating an attack. If steroids aren’t working, should PLEX, IL-6 inhibition, or complement inhibition already be considered when appropriate? The workup for long-term immunotherapy, vaccination review should begin before discharge. The inpatient admission is often the best opportunity to change the trajectory of a chronic neuroimmunologic disease. Let’s not waste it. Neuroimmunologists want to see these patients. Many of us will make every effort to expedite urgent referrals if contacted directly. Patient advocacy organizations such as the National MS Society and disease-specific organizations for NMOSD and MOGAD can also help identify local specialists. Let’s use those resources instead of simply writing, “Follow up with neuroimmunology ASAP.”

Infectious myelopathies are likely under-recognized. As neurologists, we should keep them in the differential diagnosis of every myelopathy, as missing them can mean missing a treatable cause! Here, we share our neurological-perspective review, now in Current Opinion! #Neurology

IVIg for CIDP: Start with the loading dose (2 g/kg), then give 1 g/kg every 3 weeks. Schedule a follow-up visit in 9-12 weeks. Treatment response should be assessed using the INCAT score (at least 1 point improvement) and/or the MRC sum score (>2 points). If there is no response, reconsider the diagnosis, as it is unlikely to be CIDP. Do not use symptom improvement or “halted progression” as proof of response. Refer to a @gbscidp center of excellence if there is no response or if there are doubts about the diagnosis.

This is an excellent review on this topic ! Persistent diagnostic uncertainty in suspected neuroinflammation should trigger genetic testing. You can’t diagnose what you don’t test.
This review examines some of the monogenic disorders that can masquerade as neuroinflammatory phenotypes: hubs.la/Q03WVsG10 #NeuroTwitter #NeuroX
