First phase 3 RCT demonstrating superiority of a chemotherapy regimen (PAXG over mFOLFIRINOX) in the neoadjuvant setting for resectable and BR PDAC. Primary endpoint ➡️1yr-EFS✅, waiting for mature OS data. 🇮🇹 effort led by @SanRaffaeleMI, funded by patient advocacies.

✌️Proud of being part of this wonderful research group‼️A relevant step ahead on a long way to go: unraveling the intricacies of PDAC biology @vincorbo @SalviaRobi @CLuchini10 @BottonChampPanc @AndreaSottoriva @Valsecchifdn @PanCAN @Run4Purple @AISP_Pancreas @P_C_E_ @letswinpc
🚨🚨Out in @Nature, led by @vincorbo: ecDNA is a major source of high-level focal amplification in key oncogenes and a major contributor of MYC heterogeneity in PDAC. MYC dosage affects cell morphology and dependence on stromal niche factors. nature.com/articles/s41586-0…

Thank you @TFStoop for putting this together🤩 great int’l collaboration under the @EAHPBA umbrella 💪
In @JAMAOnc our 🌎 @EAHPBA study on adjuvant chemo (AT) in 742 resected localized #PDAC patients following NAT #FFX (40 centers): 📌OS ⬆️ by AT; … only with multi-agent AT ⚠️ 📌Possibly less effect of AT if >=8c NAT FFX, RECIST response, ypN0 & head tumor 📣 📯RCT🟰🔑

Comprehensive pathologic review of post-neoadjuvant pancreatectomy specimens identified 10 morphologic subtypes, which correlate with survival. Also, intratumor spatial heterogeneity is associated with bad prognosis.

Simple clinical parameters (ASA class, tumor size and serum Ca 19.9) help predict futile upfront pancreatectomy in resectable PDAC. Quantitative risk estimation (online calculator) + simplified dichotomous criteria. Not a perfect model, no fancy biomarkers, but pragmatic stuff.
🚨The MetroPancreas model can help predict a futile upfront resection for PDAC (death or recurrence within 6mo) and select parients for neoadjuvant strategies using simple clinical parameters. Check it out ➡️ jamanetwork.com/journals/jam… Online calculator ➡️ aicep.website/metropancreas/

🔥🔥 hot off the press in the @AnnalsofSurgery 🫸 Chasing neck margin to obtain R0 in post-neoadjuvant PD does not translate into improved outcomes ⚠️Positive neck margin associated with non-mitigable prognostic factors, including other margin involvement, and N+ disease
Do you check pancreatic neck margin for frozen section in post-neoadjuvant PD❓In our new paper with @SanRaffaeleMI + @MGHSurgery, conversion of an initially positive neck margin by additional resection was not associated with improved outcomes. journals.lww.com/annalsofsur…
